Students that want to participate in any athletic activity must complete the following steps:
Step 1: Sport Registration:
Parents must sign into parent portal and review/acknowledge all consents, then "sign-up" the student for the sport of choice.
Step 2: Submit the following forms :
Completed State Physical Exam packet (physical valid for one year)
History update forms (must be submitted every season)
To submit electronically: scan document (must be full size) send to

In person: Bring the completed physical packet to the High School Main office, hand it to the secretary and tell her it's for Mr.Alfonso. Make sure to make a copy of it and keep it!
Step 3: Covid-19 Forms:
Print, complete, and submit both forms to coach on first day of practice.
Covid-19 Questionnaire will be submitted once.However, the Covid-19 Daily Prescreening Form must be printed and completed daily, prior to the start of practice.
