1). Private Physician
Make an Appointment ASAP
Print the Sport Physical Packet from link below
See Sport Physical Packet Explanation below for directions on how to complete the forms
Once the forms are complete, turn them in to the HS main Office
Send email to cmartone @gboe.org, state physical has been turned in name, grade and sport
Keep a copy of all your paperwork!!
Your paperwork will be submitted to the School Physician for approval and clearance
This process takes about 1 week

2). School Physician 
Must attend one of the dates below (Plan Ahead)
Print Sports Physical Packet from link below
See sport Physical Packet Explanation below for directions on how to complete the forms
Report to the HS New Gym on the date assigned for your team
Must bring the Sport physical packet and a pen
Make sure the history form is completely filled out, students with incomplete paperwork will be sent home
Students will be seen on a first come first serve basis

Paper work that needs to be printed, completed, and submitted:

1). Sport Physical Packet Explanation 
        Health History Page (Page 1)
       Make sure top section of this form is complete
       Must answer Allergy question
       Must check yes or no for each question, do not leave any blank
       If you checked off yes to any question please explain why on the lines provided
       Parent must sign at the bottom of the page

        Physical Exam Page (page 2-3)
       Will be completed by the Physician
       Top of the form must be fully completed
       Must have a valid eye exam 
       If the doctor crosses it out and writes "wears glasses/contacts", this will not be accepted
       Must have 3 Physician Signatures and an office stamp

2). Health History Update Form
      Print the form from the link below
      Must be completed in addition to the Sports Physical Packet
      Must be done every season
      Only list injuries or changes that have happened within the last year
